MEMBER RIGHTS AND RESPONSIBILITIES

 

Member Rights

 

·    A member has the right to considerate and respectful care.

·        A member has the right to receive information about the illness, course of treatment and prospects for recovery in terms that the member can understand.

·        A member has the right to receive as much information about any proposed treatment or procedure, regardless of cost or benefit coverage, as the member may need in order to give informed consent or to refuse the course of treatment.  Except in emergencies, this information shall include a description of the procedure or treatment, the medically significant risks involved, alternate courses of treatment or non-treatment and the risks involved in each and to know the name of the person who will carry out the procedure or treatment.

·        A member has the right to actively participate in decisions regarding medical treatment, including the right to refuse treatment.

·        A member has the right to privacy concerning their medical care.

·        A member has the right to confidential treatment of all communications and records pertaining to his/her care.

·        A member has the right to reasonable responses to any and all reasonable requests made for service.

·        A member has the right to reasonable continuity of care.

·        A member has a right to be informed of continuing health care requirements.

·        A member has a right to request an Advanced Directive and will be directed to agencies that will assist in completing all forms.

·        Members have the right to receive information about the medical group, its services, its practitioners and providers, and members' rights and responsibilities.

·        Members have the right to file complaints or appeals about the group or the care provided.

 

 

Member Responsibilities

 

·        The member has the responsibility to provide accurate and complete information concerning his present complaints, past medical history and other matters relating to his health.

·        The member is responsible for following the treatment plan established by his/her physician, including instructions of nurses and other health professionals as they carry out the physician’s orders.

·        The member is responsible for keeping appointments and for notifying the office or physician when he is unable to keep the appointment.

·        The member is responsible for his actions should he refuse treatment or not follow the physician’s orders.

·        The member is responsible for assuring that the financial obligations of his care are fulfilled.

·        The member is responsible for being considerate of the rights of other members and office personnel.
